COMMUNITY CARE

Oak House Homecare Ltd have been operating a Community Care Agency for twelve years, supplying care services to adults and older people in their own homes.

The agency head office is situated in Olivemede, one of the Residential Care Homes owned by the company. The Manager and Administrator are based there during normal working hours and an on-call system is in operation for service users or their families for emergency contact.

The agency offers a service for older people, visual and sensory impaired, physically and learning disabled individuals, and is continually diversifying the service to incorporate the changing needs of the service user.

We assist to establish a caring, safe, happy and healthy plan for all service users, whilst encouraging individual independence, to help service users maintain and develop their own sense of personal worth and quality of life.

The care needs, wishes, preferences and personal goals for each individual service user are recorded in their personal service care plan following a thorough assessment from a home visit. The Manager contacts the person within two weeks to ensure they are satisfied with the service and support provided.
